At APLA Health, we are committed to providing you with the highest quality, patient-centered care. If we refer you to any specialists, we will also coordinate care with them.
When you first visit us, we may ask if you’ve seen any other providers for your care. This way we can get the most complete and accurate health history and serve you best.
What to Bring to Your First Medical Appointment
When you come see us for your first medical appointment, make sure you bring:
- Your insurance information, or your Medi-Cal or Medicare card. If you do not have insurance we can help you find a plan that fits your needs.
- A copy of your living will/advance health care directive, if you have one
- A list of your current medications, with doses, if known. Please bring the medicine bottles or containers with you if you can.
- Proof of income (pay stub, tax return, etc.)
- Proof of address
APLA Health charges a sliding-scale fee based on ability to pay.
