Home Health Care

Our Home Health program offers in-home services to individuals with symptomatic HIV/AIDS who need assistance to maintain independent living.

The program provides in-home health care, including nurse and social work case management, as well as support such as attendant care, homemaker services, and counseling services.  Other needed services may be provided directly, or coordinated by the case management team. Services are available in English and Spanish.

SERVICES WE PROVIDE

Assessment

Clients will be evaluated for their physical, psychological, social, environmental, and financial status to determine their service needs. A face-to-face re-assessment will be completed on a regular basis.

 

Registered Nurse Case Management Services

Clients will receive home visits based on their condition on a regular basis. The Nurse Case Manager provides medical case management, patient education, assistance with treatment adherence, and oversees the care plan with regard to medical and personal care needs.

 

Social Work Case Management Services

Clients will receive counseling assistance with psychosocial, financial, housing, and related concerns. The social worker may visit in conjunction with the nurse case manager, or during separate appointments.

Attendant Care Services

Clients may receive assistance by an in-home health aide or nurse assistant certified by the California Department of Health Services. Under the direct supervision of a registered nurse (RN), the attendant will provide personal care (bathing, grooming, etc.), and comfort care.

 

Homemaker Services

Clients may receive assistance by a homemaker who will assist with general household activities and in the maintenance of independent living.

 

Counseling

Clients may receive the services of a therapist to assist with their emotional and mental health needs related to living with HIV/AIDS.

WHAT ARE THE REQUIREMENTS?

  • Clients must be a resident of Los Angeles County and have HIV/AIDS with current symptoms related
    to HIV/AIDS or treatment of the disease.
  • Clients must have a California licensed physician provide a medical diagnosis form certifying their current HIV
    status within 45 days of enrollment. We will assist clients in obtaining this from their doctor.
  • Clients must meet criteria set by the service provider, the County Department of Public Health, Division of HIV
    and STD Programs, and the State Office of AIDS guidelines. This may include a functional needs assessment, reasonably
    safe and stable housing, have a physician willing to take responsibility for medical care, and other criteria depending
    upon funding source and service provider.
